Safety reports can be submitted by either by;
· online (preferable) using the Club’s online safety management software SHEQ2GO (Safety, Health, Environment and Quality to Go)

Club Member User Login (this is the member or staff member’s email address as recorded in the CYC Revsports system.
Password: a default password as advised by the Club that can be managed by the member
· in writing on the Safety Report Form, copies of which are available either online at the Club’s website under Policies and Procedures ( see link https://www.chelseayachtclub.com.au/about/downloads ) or beside the Safety Report Box located near the main Race Board.
In writing reports may be either submitted electronically by email to safety@chelseayachtclub.com.au or placed in the Safety Report Box located near the main Race Board.
Confidential reports can be submitted by either ticking the “confidential” tick box when submitting via the online software SHEQ2GO or marking the submitted email or paper submission as confidential.
Anonymous reports can be submitted by either ticking the “anonymous” tick box when submitting via the online software SHEQ2GO or marking the submitted email or paper submission as anonymous.
The Safety Officer will save any reports submitted either by email or hardcopy in the Club’s online safety management software SHEQ2GO.
The following are events that must be notified to the Safety Officer in a Safety Report:
· Injuries
· Equipment damage
· Near misses
· Breakdown of a standard operating procedure
